One-Time Enrollment Fee: $575 for adults; $475 for children.
Commitment: Minimum of 6 months, with monthly billing.
Loyalty Credit: Earn a $25 credit for attending scheduled cleanings, usable towards treatments, supplements, whitening, etc.

Emergency Assessment: Includes one emergency exam and one necessary 2D imaging scan during the visit.
